From the Nurse: Medical Forms and Medication
In order to be ready for the first day of school, please make sure all health paperwork and/or medications are dropped off at Fairview. You are welcome to stop by between 8:00 AM and 4:00 PM Monday-Friday, fax health forms to our office at 847-357-5701, or email our school nurse, Mrs. Straub, at CynthiaStraub@sd54.org
Immunizations:Immunizations: All immunizations must be current according to the requirements of the Illinois School Code. Students who fail to comply will be excluded from school. https://www.sd54.org/page/immunization-requirements . Information regarding Influenza (flu) and Meningococcal Disease and the vaccine can be accessed below or on the Illinois Department of Public Health website.
Physical Exam: The Illinois School Code requires that children entering preschool, kindergarten (or first grade if kindergarten was not attended) and sixth grade must have had a physical exam within one year prior to starting that grade level. Students of any age transferring from a school outside Illinois must also have had a physical within the last year.https://www.sd54.org/page/health-services
